• Conduct thorough assessments and develop individualized treatment plans that address the psychological and emotional needs of patients.
• Utilize evidence-based screening tools and clinical methodologies to provide high-quality care.
• Collaborate with healthcare teams to create and implement crisis intervention plans, risk assessments, and positive behavior support strategies.
• Maintain accurate and timely documentation in compliance with regulatory guidelines, ensuring all records are prepared for licensing and accreditation processes.
• Act as a liaison for patients to access relevant community resources, state agencies, and service systems.
• Engage in ongoing professional development activities, including attending relevant conferences and participating in community outreach initiatives.
• Foster communication among team members and community partners, promoting the mission of the agency while enhancing service delivery.

Our primary focus is on patients receiving hospice care, encompassing a wide range of psychological issues associated with terminal illnesses. You will provide support to patients and their families, addressing concerns surrounding grief, loss, and adjusting to end-of-life transitions. Your role will be instrumental in alleviating emotional distress and enhancing the overall quality of life for those in our care.

• A Master’s Degree in Social Work (MSW) from an accredited institution.
• A valid LCSW license in good standing with the appropriate state licensing board.
• A minimum of two years of supervised clinical experience in a healthcare or hospice setting.

• Experience in hospice or palliative care environments focusing on end-of-life mental health issues.
• Familiarity with telehealth platforms and virtual care delivery.
• Proficiency in collaborating with multidisciplinary teams.
