Job description

Overview
We are seeking a compassionate and detail-oriented Physician Assistant (PA) -Active and unrestricted New York and New Jersey license (required) to join our growing telehealth team. In this role, you will provide high-quality, patient-centered care through virtual visits, helping expand access to healthcare while maintaining clinical excellence. You will play a key role in delivering consistent, proactive care to patients, particularly those managing chronic conditions, by conducting comprehensive virtual assessments and ensuring continuity of care. This position requires strong clinical judgment, excellent communication skills, and the ability to build meaningful patient relationships in a remote setting. As part of a collaborative care team, you will work closely with our Chronic Care Management Team, Remote Patient Monitoring Team and support staff to optimize patient outcomes, reduce hospitalizations, and improve overall quality of life for the populations we serve.

Duties

  • Conduct telehealth visits with patients enrolled in chronic care management program

  • Monitor and manage chronic conditions such as diabetes, hypertension, COPD, and heart disease

  • Review patient data, labs, and care plans to ensure continuity of care

  • Collaborate with Chronic Care Management Team and Remote Patient Monitoring Team to monitor health and symptoms.

  • Provide patient education, coaching, and support for disease management

  • Document all patient interactions accurately in the electronic medical record (EMR)

  • Ensure compliance with CCM billing requirements and telehealth regulations

  • Maintain accurate and timely electronic medical records (EMR) documentation

  • Obtain and document medical histories and perform assessments

Requirements

  • Current State license in good standing and/or certification as a Physician Assistant (PA-C)

  • Active and unrestricted New York and New Jersey licenses (required)

  • Additional licensure in Florida and Pennsylvania strongly preferred

  • Minimum of 5 years of clinical experience (Primary Care, Internal Medicine), telehealth experience a plus!

  • Strong clinical decision-making and diagnostic skills

  • Excellent communication and bedside manner in a virtual setting

  • Comfortable using telehealth technology and electronic health records

  • Ability to build rapport with patients in a virtual environment

  • Strong organizational and time-management skills
