• Conduct thorough psychosocial assessments through interviews, observations, and reviews of medical records to identify emotional, social, and environmental factors influencing patient recovery.
• Provide individual and group counseling, offering emotional support to patients and their families during the rehabilitation process.
• Collaborate with the rehabilitation team to develop tailored treatment plans, ensuring that psychosocial needs are integrated into patient care strategies.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation programs, and affordable housing options.
• Deliver education and resources to patients and families about post-rehabilitation services and community support available for ongoing recovery.
• Participate actively in discharge planning and facilitate smooth transitions from rehabilitation to home or other care settings, ensuring patient preparedness and continuity of care.

Your work will focus on a diverse patient population with various backgrounds and rehabilitation needs. You will support individuals who may be dealing with the psychological impacts of significant changes in their lives due to health issues, promoting emotional well-being and resilience through therapeutic techniques.

• A Master's degree in Social Work from a program accredited by the Council on Social Work Education.
• An active state license as a Licensed Clinical Social Worker (LCSW) in Kansas.
• Strong understanding of psychosocial dynamics, adjustment issues, grief, and coping mechanisms relevant to rehabilitation.
• Exceptional communication and interpersonal skills to foster relationships with patients and interdisciplinary team members.

• Previous experience within a rehabilitation or medical setting, providing valuable insights into the unique challenges faced by patients in recovery.
