Molina Behavioral Health Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. Work Environment

This position is fully remote, which allows for flexibility whilemaintaining a strong connection to our hospice center's team and resources. Our work environment is collaborative, leveraging digital tools to ensure effective communication and support among interdisciplinary teams. While you will primarily work from home, occasional meetings or training sessions may take place at our Overland Park facility.

Key Responsibilities

• Conduct thorough psychosocial assessments by engaging with patients through interviews, observations, and comprehensive reviews of medical records. This critical process evaluates social, emotional, and environmental factors that may influence their recovery.
• Provide compassionate counseling and emotional support to patients and their families. This includes addressing various adjustment issues and promoting coping strategies that enhance emotional well-being.
• Collaborate effectively with physical and occupational therapists, rehabilitation specialists, and other healthcare providers to develop, implement, and refine individualized treatment plans tailored to each patient's unique psychosocial needs.
• Guide patients in accessing essential community resources such as support groups, vocational rehabilitation services, and housing assistance, facilitating a smoother transition to their post-rehabilitation lives.
• Engage in discharge planning by evaluating each patient's readiness to transition back to their home or other care settings, ensuring a comprehensive support network is in place.

Patient Population / Client Focus

You will work with a diverse patient group that includes individuals recovering from serious medical or psychological conditions like stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. This role demands sensitivity to the unique challenges faced by each patient, tailoring your approach to meet their specific needs and fostering their emotional recovery alongside physical rehabilitation.

Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong interpersonal and communication skills, with an emphasis on building rapport and trust with patients and families.

Preferred Experience

• Experience in a rehabilitation or medical setting is highly desirable to equip you with a stronger understanding of the complexities of patient recovery.
• A solid foundation in adjustment issues, grief counseling, and coping mechanisms will enhance your effectiveness in this role.

Compensation and Benefits

The salary for this position ranges from USD 75,000 to 100,000 annually, commensurate with experience and qualifications. In addition to a competitive salary, we offer a sign-on bonus to welcome our new team member.
