As a social worker in a hospice center, you will engage with patients and families through virtual channels, ensuring ongoing support for emotional and psychological needs. • Provide psychotherapy and mental health services via telehealth platforms to patients facing various mental health conditions.
• Evaluate patients' mental health through comprehensive assessments, focusing on their emotional, behavioral, and social functioning.
• Develop individualized treatment plans that employ evidence-based practices and consider cultural factors influencing patient care.
• Work collaboratively with interdisciplinary teams, including psychiatrists and primary care providers, to enhance patient support and ensure comprehensive care.

The ideal candidate will work with a diverse range of patients, including those experiencing anxiety, depression, post-traumatic stress disorder (PTSD), and end-of-life issues. • A Master’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
