• Conduct thorough assessments of clients to determine their unique mental health needs.
• Regularly communicate with supervisors, direct care staff, family members, and other parties to discuss client progress and necessary adjustments in care.
• Develop and provide a tentative schedule for appointments and home visits, ensuring timely follow-ups and monitoring of client progress.
• Coordinate necessary home visits to gather detailed observations, conduct chart reviews, and analyze data critical to patient care.
• Implement and monitor the effectiveness of program plans, ensuring that interventions are appropriate and beneficial for clients.
• Participate in interdisciplinary team meetings, contributing insights and recommendations for patient care.
• Generate and complete monthly and quarterly progress notes to document client outcomes and interventions.
• Organize and lead training sessions for staff on program implementation and ensure compliance with documentation protocols.
• Provide continuous support through follow-up interviews and home visits to promote client stability and progression.
• Maintain organized records that guarantee accessibility of information to authorized individuals.
• Act as a liaison with external community resources and the mental health community to aid in development objectives.
• Contribute to the creation of training materials aimed at improving program efficacy and staff competency.

In this role, you will engage with a diverse client population, including both adults and youth experiencing various mental health challenges. Your work will be integral in fostering their recovery and well-being through personalized care strategies, focusing on areas such as family dynamics, trauma recovery, and substance misuse. You will play a vital role in guiding clients through their therapeutic journeys, preparing them for better integration into their communities.

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Maryland.
• A minimum of two years' experience in counseling, preferably within a clinical context.
• Strong foundation in legal and ethical guidelines governing mental health practice.
• Exceptional interpersonal and communication skills, with a proven ability to build rapport with diverse clientele.
• Demonstrated ability to work collaboratively with a healthcare team to enhance overall patient outcomes.
• Proficiency in EMR software and related technologies for efficient patient management.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse is highly preferred.
• Experience implementing therapeutic interventions in a remote or hybrid care environment.

• Competitive salary range of USD 90,000 – 100,000 annually, commensurate with experience and caseload.
