• Conduct in-depth psychosocial assessments through various methods, including interviews, observations, and examination of medical records to determine the emotional and social needs of each patient.
• Provide compassionate counseling and support to patients and their families, helping them to address adjustment issues and enhance emotional well-being as they transition through their rehabilitation journey.
• Collaborate with a diverse rehabilitation team to create individualized treatment plans that integrate psychosocial and therapeutic needs, ensuring comprehensive care delivery for each patient.
• Assist patients in accessing community resources, such as support groups, vocational rehabilitation programs, and housing assistance, facilitating their reintegration into the community.
• Participate in discharge planning to support smooth transitions for patients returning home or moving to another care setting, ensuring continuity in care and resources.
• Educate patients and families about available community resources and post-rehabilitation services, empowering them with information to support their recovery process.

In your role as a Licensed Clinical Social Worker, you will focus on a diverse patient population that includes individuals recovering from conditions such as stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. This role is crucial in addressing not only the psychological aspects of recovery but also the social factors that can impact a patient’s rehabilitation process, thereby fostering both emotional resilience and functional independence.

• A Master's degree in Social Work from an accredited program is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) in New York is mandatory.
