As a Full-Time Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough psychosocial assessments to evaluate the emotional, social, and environmental factors affecting patients' recovery through interviews and medical record reviews.
• Providing empathetic counseling and support for patients and their families, addressing issues related to adjustment, grief, and emotional well-being.
• Collaborating with the rehabilitation team to develop individualized treatment plans tailored to the psychosocial needs of each patient.
• Guiding patients in accessing vital community resources, such as support groups, vocational rehabilitation services, and housing assistance, to enhance their post-rehabilitation transition.
• Offering education and ongoing support to patients and families about available community resources and post-rehabilitation services, thereby promoting long-term success.
• Participating actively in discharge planning to facilitate transitions to home or other care environments, ensuring continuity of care and support.

Your work will primarily focus on a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. You will play an essential role in addressing their emotional and social needs, thus facilitating a smoother rehabilitation process and contributing to positive health outcomes.

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• An active state licensure as a Licensed Clinical Social Worker (LCSW).
• A solid understanding of adjustment issues, grief, coping mechanisms, and family dynamics.
• Excellent communication skills, with the ability to convey complex concepts clearly and empathetically.

While not mandatory, preferred experience includes:

• Previous work experience in rehabilitation or medical settings, contributing to the understanding of the complexities involved in patient recovery.
