• Direct psychoeducational and process-oriented therapy groups, facilitating meaningful discussions and interventions.
• Conduct individual therapy sessions tailored to patient needs, employing evidence-based practices and therapeutic techniques.
• Perform thorough clinical assessments upon admission to understand each patient's unique mental health needs and develop personalized care plans.
• Communicate effectively with the Utilization Review and Billing Departments regarding authorization processes and payment structures.
• Initiate discharge planning on the day of patient admission to ensure a seamless transition out of hospice care.
• Lead discharge sessions that equip patients and their families with vital coping skills, transition plans, and appropriate resources to facilitate their ongoing recovery.
• Confirm discharge appointments with patients and their loved ones, reinforcing support systems before exit from care.
• Maintain ongoing communication with referral sources and outpatient providers throughout the admissions and discharge process to support continuity of care.

This role focuses on serving individuals in hospice care, where emotional and psychological support is crucial. Patients often deal with complex emotional responses to terminal illness, necessitating a compassionate and skilled approach to care. As an LCSW, you will assist patients and their families in navigating their challenges while fostering an environment of understanding and respect.

• A Master’s degree in Social Work from an accredited institution.
• An active and unrestricted license as a Clinical Social Worker (LCSW) in the state of California.
• Exceptional written and verbal communication skills, with the ability to convey complex concepts clearly and compassionately.
• Proven ability to establish and maintain appropriate boundaries with clients as part of professional conduct.
• Strong organizational skills with the ability to complete and document services in a timely manner, adhering to compliance requirements.

• Experience working in hospice, palliative care, or similar clinical settings is highly desirable.
• Familiarity with case management practices and discharge planning is advantageous.
• Proficiency in working with electronic medical records (EMR) and software commonly used in healthcare settings.
