• Conduct thorough client assessments to determine individual needs, establishing care plans that address both psychological and emotional aspects of the hospice journey.
• Implement effective behavior management strategies, working closely with clients to create coping mechanisms that can ease their transition.
• Facilitate individual and group counseling sessions, addressing a range of topics from grief and loss to life legacy discussions, fostering a safe space for expression.
• Provide education to families on how to support their loved ones through end-of-life care, ensuring they feel empowered and informed.
• Complete timely and accurate clinical documentation in accordance with organizational standards, utilizing electronic health record (EHR) systems to ensure compliance and continuity of care.
• Collaborate with a multidisciplinary team, including nurses, physicians, and chaplains, to ensure holistic care that encompasses medical, spiritual, and emotional support.

In this role, you will be primarily working with terminally ill patients and their families, providing comprehensive support during a delicate phase of life. Your work will focus on helping clients cope with the emotional toll of a terminal diagnosis, while also partnering with families to navigate the complexities of end-of-life decisions and grief.

• A Master’s degree in Social Work or a related field from an accredited institution.
• An active Licensed Clinical Social Worker (LCSW) license in California.
• At least two years of relevant clinical experience, ideally in hospice or palliative care settings.
• Strong organizational skills and the ability to manage multiple cases effectively.
• Excellent interpersonal communication abilities that foster a supportive environment for clients and families.

• Experience in grief counseling or working with end-of-life issues, particularly in a hospice setting.
