• Conduct thorough initial and ongoing assessments to identify the unique needs of each patient.
• Maintain open lines of communication with supervisors, direct care staff, and family members regarding patient progress, ensuring comprehensive support.
• Create and update a tentative schedule for appointments, home visits, and observation sessions to monitor patient development effectively.
• Coordinate logistics for home visits, including in-home observations, chart reviews, and staff interviews to ensure all client requirements are met.
• Implement and monitor the effectiveness of individualized program plans, adapting strategies as needed based on patient feedback and progress.
• Participate in interdisciplinary team meetings, sharing insights and collaborating toward best practices for patient care.
• Complete and document monthly and quarterly progress notes to track advancements and setbacks in patient treatment.
• Organize and facilitate training sessions to ensure all team members are sufficiently prepared to implement program plans and understand documentation practices.
• Provide follow-up support through interviews, home visits, and observations, reinforcing the commitment to each patient's ongoing needs.
• Maintain accurate patient records, ensuring that essential information is accessible to relevant stakeholders.
• Act as a liaison within the mental health community, cultivating relationships to enhance our developmental goals.
• Assist in the design and improvement of training materials and standard operating procedures for enhanced program efficacy.

As an LCPC with Open Doors Counseling, you will have the privilege of working with a diverse client population, including adults, youth, families, and couples. You will tailor your approach to meet the complexities of various mental health disorders and challenges while emphasizing trauma-informed care and empathy. • Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
• A minimum of two years of experience in a counseling role within a clinical setting, demonstrating expertise in mental health interventions.
• Preferred specialization in areas such as family therapy, couples therapy, trauma, or substance misuse.
• Strong understanding of legal and ethical standards governing counseling practices.
• Excellent interpersonal and communication skills, with a proven ability to build rapport with a variety of clientele.
• Demonstrated capacity to work collaboratively within a healthcare team.
• Proficient in using Electronic Medical Record (EMR) software and related technologies for patient documentation.

• Prior experience in a multi-disciplinary team setting, providing comprehensive care to diverse populations.
• Familiarity with evidence-based practices and therapeutic modalities relevant to client needs.
• Previous involvement in capacity-building initiatives or training program development is beneficial.

• Annual salary range from USD 90,000 to USD 110,000, commensurate with experience and caseload availability.
