This position will be fully remote, offering flexibility to you as you manage your schedule and client engagements. While you will be working individually, you will be a vital part of an interdisciplinary team that frequently collaborates to assess and support each client's unique needs. • Conduct thorough and ongoing assessments of client needs through initial intake appointments and continuous evaluations.
• Maintain open lines of communication with supervisors, fellow counselors, family members, and other stakeholders to monitor and discuss client progress.
• Develop and provide a tentative schedule of appointments, including virtual meetings and home visits, to supervisors, ensuring transparency in your workflow.
• Coordinate home visits and in-home observations to facilitate comprehensive treatment planning and monitoring.
• Implement and monitor treatment plans for clients, ensuring adherence to established clinical goals and protocols.
• Engage in regular interdisciplinary team meetings to collaborate on care strategies and patient outcomes.
• Document client progress through monthly and quarterly notes, providing vital feedback to inform care adjustments.
• Organize and execute training for team members to enhance their capabilities in program plan implementation and documentation practices.
• Provide ongoing support through follow-up interviews and additional home visits to assess clients’ evolving needs.
• Act as a liaison within the mental health community to promote and foster growth initiatives for behavioral health programs.
• Contribute to the development of training materials aimed at refining therapeutic approaches within the organization.

Your role will involve working with a diverse client demographic, addressing a variety of mental health concerns ranging from anxiety and depression to trauma and substance misuse. We cater to both adult and youth populations, thereby requiring a versatility in counseling styles and approaches. Your expertise will be crucial in tailoring treatment strategies that resonate with the individual needs of each client.

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.
• Possession of an active and unrestricted Clinical Professional Counselor (LCPC) license in Illinois is essential.
• A minimum of two years of direct counseling experience, demonstrating a strong background in clinical practice.
• Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly desirable.
• A thorough understanding of the legal and ethical standards governing mental health practices.
• Demonstrated interpersonal and communication skills, allowing for effective rapport with clients from diverse backgrounds.
• Ability to work collaboratively within a healthcare team, contributing to shared decision-making and treatment planning.
• Experience with Electronic Medical Records (EMR) software and other related technologies.

• Experience working in a community health setting, particularly within a remote counseling framework.
• Familiarity working with a variety of mental health issues, adapting treatment modalities to fit client needs.
• Prior exposure to interdisciplinary collaborative care models.
