Key Responsibilities
  • Conduct thorough and ongoing assessments of client needs through initial intake appointments and continuous evaluations.
  • Maintain open lines of communication with supervisors, fellow counselors, family members, and other stakeholders to monitor and discuss client progress.
  • Develop and provide a tentative schedule of appointments, including virtual meetings and home visits, to supervisors, ensuring transparency in your workflow.
  • Coordinate home visits and in-home observations to facilitate comprehensive treatment planning and monitoring.
  • Implement and monitor treatment plans for clients, ensuring adherence to established clinical goals and protocols.
  • Engage in regular interdisciplinary team meetings to collaborate on care strategies and patient outcomes.
  • Document client progress through monthly and quarterly notes, providing vital feedback to inform care adjustments.
  • Organize and execute training for team members to enhance their capabilities in program plan implementation and documentation practices.
  • Provide ongoing support through follow-up interviews and additional home visits to assess clientsโ€™ evolving needs.
  • Act as a liaison within the mental health community to promote and foster growth initiatives for behavioral health programs.
  • Contribute to the development of training materials aimed at refining therapeutic approaches within the organization.
Patient Population / Client Focus

Your role will involve working with a diverse client demographic, addressing a variety of mental health concerns ranging from anxiety and depression to trauma and substance misuse. Qualifications
