Key Responsibilities
  • Conduct comprehensive and ongoing assessments to tailor therapeutic approaches that adequately meet the needs of clients.
  • Facilitate effective communication among supervisors, direct care staff, family members, and guardians regarding individual progress and treatment plans.
  • Design and submit a tentative schedule for appointments, home visits, and observations to ensure clients' therapeutic goals are met timely.
  • Coordinate home visits to conduct necessary observations, chart reviews, and staff interviews, ensuring a thorough understanding of clients' environments and challenges.
  • Implement and monitor the effectiveness of program plans, making necessary adjustments based on client feedback and progress.
  • Participate in interdisciplinary team meetings to collaborate and strategize the best approaches for individual client needs.
  • Maintain accurate and up-to-date records, including monthly and quarterly progress notes, to streamline client information access.
  • Serve as a liaison between the mental health community and the public to promote support for developmental goals.
  • Provide training and support to staff members to enhance their competence in implementing program plans and documentation.
  • Conduct follow-up interviews, home visits, and observations to ensure sustained support for ongoing client needs.
