Key Responsibilities

  • Provide individualized counseling and support to patients and their families, helping them navigate adjustment issues related to their health and well-being.
  • Collaborate closely with physical and occupational therapists, nurses, and physicians to develop customized treatment plans that address patients' psychosocial needs alongside their medical care.
  • Assist patients in accessing vital community resources such as counseling services, vocational rehabilitation, and housing assistance, ensuring a smooth transition post-rehabilitation.
  • Educate patients and families about available community resources and post-rehabilitation services to empower them in their ongoing recovery.
  • Engage in discharge planning processes that facilitate seamless transitions to home or alternate care settings, ensuring continuity of care.
Patient Population / Client Focus

You will primarily work with a diverse patient population ranging from individuals recovering from acute medical events to those living with chronic disabilities. Supporting this demographic, you will address varied emotional and psychological needs, promoting resilience and coping mechanisms to enhance their quality of life.

Qualifications
  • A Master’s degree in Social Work from an accredited program.
  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
