This role is fully remote, allowing for flexibility while still being part of a committed team dedicated to rehabilitation. We operate with a strong focus on patient-centered care in a supportive environment, encouraging collaboration among rehabilitation professionals. Regular business hours will be the standard, with occasional evening and weekend shifts depending on patient needs and assigned caseloads.

• Conduct thorough psychosocial assessments through patient interviews, observations, and reviews of their medical history to evaluate emotional, social, and environmental factors impacting their recovery and rehabilitation journey.
• Provide individualized counseling and support to patients and their families, helping them navigate adjustment issues related to their health and well-being.
• Collaborate closely with physical and occupational therapists, nurses, and physicians to develop customized treatment plans that address patients' psychosocial needs alongside their medical care.
• Assist patients in accessing vital community resources such as counseling services, vocational rehabilitation, and housing assistance, ensuring a smooth transition post-rehabilitation.
• Educate patients and families about available community resources and post-rehabilitation services to empower them in their ongoing recovery.
• Engage in discharge planning processes that facilitate seamless transitions to home or alternate care settings, ensuring continuity of care.

You will primarily work with a diverse patient population ranging from individuals recovering from acute medical events to those living with chronic disabilities. Supporting this demographic, you will address varied emotional and psychological needs, promoting resilience and coping mechanisms to enhance their quality of life.

• A Master’s degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of psychological adjustment and coping mechanisms related to trauma and chronic illness.
• Exceptional communication and interpersonal skills to effectively build rapport with patients, families, and the rehabilitation team.

• Experience in a rehabilitation or medical setting is highly desirable, as is familiarity with community resource navigation.
• Previous exposure to interdisciplinary treatment planning and collaboration.

The salary range for this position is competitive, ranging from USD 75,000 to 100,000 annually, along with a sign-on bonus. We offer a comprehensive benefits package that includes:
