Position Overview

Thrive Compassionate Counseling is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This position plays a vital role in our patients' rehabilitation journeys by addressing their emotional, social, and practical needs while promoting well-being and integration into the community.

Work Environment

This position is primarily remote, providing flexibility while ensuring you are part of an interdisciplinary team focused on delivering high-quality care. Key Responsibilities
  • Conduct comprehensive psychosocial assessments through patient interviews, observations, and detailed medical record reviews. These evaluations help ascertain the emotional, social, and environmental factors that may be impacting each patient’s recovery process.
  • Provide individualized counseling and emotional support to patients and their families, particularly in addressing adjustment issues, grief, and coping strategies.

  • Assist patients in navigating community resources, such as integration into support groups, vocational rehabilitation, and housing assistance, to foster continued personal growth outside the clinical setting.
  • Educate patients and their families about available community resources and facilitate post-rehabilitation services that support holistic recovery.
  • Engage actively in discharge planning, ensuring smooth transitions for patients as they return home or move to alternative care settings, reflecting continuity of care.
Patient Population / Client Focus

In this role, you will be pivotal in supporting a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications
