Job description

ABOUT THE ROLE:

The RN Care Manager supports members participating in Rightway's Disease Management Programs (e.g., obesity, diabetes). The RN partners with a multidisciplinary care team to deliver personalized clinical support, care coordination, and ongoing monitoring. This role serves as the member's trusted clinical advisor with the goal of improving clinical outcomes, supporting long-term member success, and building a positive member experience.

  • Denver Hybrid role: remote Monday, Wednesday, and Friday; in-office Tuesday and Thursday
  • Shift available: 9:00 am-5:30 pm MSTย 
  • May require occasional holiday and weekend coverage based on member and program needs

WHAT YOU'LL DO:

  • Consult with members regularly to establish goals, develop and review care plans, and monitor progress towards health goals, while fostering shared decision-making through personalized support.ย ย 
  • Proactively monitor clinical risk and deploy appropriate risk mitigation interventionsย 
  • Empower members with self-management education that prevents disease progression and enables them to manage their condition outside of an acute care setting.ย 
  • Collaborate closely with the other clinicians to deliver coordinated, whole-person clinical support.
  • Identify behavioral health and social determinants of health needs and connect members to appropriate internal resources or external point solutionsย 
  • Accurately and promptly document member interaction, including but not limited to consultation and intervention notes, nursing care plan, and assessments.ย 
  • Maintain a patient-centric, empathetic, and motivational approach that supports long-term engagement and positive behavior change.
  • Protect member privacy and maintain HIPAA compliance at all times
  • Utilize provided technology effectively, including communication platforms, clinical software, and remote monitoring tools.
  • Support organizational goals for member satisfaction, clinical quality, and operational efficiency.
  • Must be able to work assigned shifts, including rotating holidays to ensure adequate member coverage.
  • Flexibility to adjust hours occasionally based on operational needs, including surge periods or emergency response situations.
  • Reliable attendance is essential for maintaining service levels and member access.
  • Remote work environment with reliable highspeed internet.
  • Ability to sit for extended periods while performing telephonic or chat assessments.
  • Use of computer, headset, and telecommunication tools for the majority of the shift.
  • Perform other duties as assigned.

WHO YOU ARE:

  • RN or BSN degree
  • Active, unrestricted RN license required; multi-state licensure required, and must live in a compact stateย 
  • Additional state licenses desired: CA, NY, MI, MN, OR
  • 5+ years of clinical experience required
  • Background in care management and/or chronic condition support required
  • Behavioral health experience is highly desired
  • Experience with GLP-1 medications and/or weight management programs is desired
  • Skilled in motivational interviewing and coaching members through sustained behavior change
  • Strong telephonic assessment, clinical judgment, and communication skills
  • Comfortable working within an app-based, technology-enabled care model

COMPENSATION: $34.00 - $40.86/hr

Compensation offered will be determined by geographic location, experience, and qualifications.
