The Home Admissions Specialist is responsible for driving an efficient intake process for home patients. A successful Home Admission Specialist will communicate well with internal and external stakeholders to gather information, meticulously document that data, and provide regular updates to patients, nurses, and DaVita Home Leadership on patients' progress in their intake journey.

ESSENTIAL DUTIES & RESPONSIBILITIES

Admissions - Partner with nephrologists, surgeons and DaVita's internal Admissions to ensure an efficient and timely home patient intake process.

Administrative - Coordinate home start activities effectively and on-schedule, including properly assigning tasks to other teammates and following up in a timely manner to ensure completion.

Communication - Lead outreach to patients and DaVita teammates at all stages of the intake process to ensure all parties are aware of upcoming responsibilities, deadlines and that patients feel supported.

Patient Tracking - Electronically create patient records to track home patient starts, as well as existing in-center patient education on home.
