You'll work closely with team members, conducting virtual consultations and meetings, which fosters a culture of collaboration and support. • Conduct thorough psychosocial assessments and evaluations to identify clients' needs, strengths, and treatment goals.
• Utilize effective behavior management strategies in individual and group counseling sessions, tailoring interventions to clients' unique situations.
• Collaborate with a multidisciplinary team consisting of physicians, nurses, and other healthcare professionals to optimize client care and treatment plans.
• Develop and implement educational resources for families to enhance their understanding of their loved ones’ developmental and mental health challenges.
• Incorporate evidence-based practices and psychotherapy techniques to achieve measurable outcomes in clients’ mental health.
• Maintain accurate and timely clinical documentation in accordance with organizational protocols and regulatory standards.

Your work will primarily center on clients dealing with mental health challenges related to end-of-life care, grief, and loss, as well as those requiring support for chronic illnesses within a hospice framework. You will engage with a diverse range of clients, including older adults and their families, providing empathy and therapeutic interventions that respect their unique cultural and personal backgrounds.

• Possession of a Master’s degree in Social Work (MSW) or a closely related field.
• A valid and active License as a Clinical Social Worker (LCSW) in the state of California.
• At least two years of post-graduate clinical experience, preferably in a healthcare or hospice environment.
• Proficiency in electronic health record (EHR) systems and relevant software for documenting client interactions and treatment plans.
• Strong organizational skills and excellent verbal and written communication abilities, with an emphasis on active listening and rapport-building.

While not required, experience with grief counseling, working with terminally ill patients, or familiarity with hospice care principles will enhance your ability to succeed in this role. Additionally, familiarity with trauma-informed care practices is highly beneficial.

The salary for this position ranges from $110,000 to $130,000 annually, commensurate with experience.
