Key Responsibilities

Patient Population / Client Focus

Your work will primarily center on clients dealing with mental health challenges related to end-of-life care, grief, and loss, as well as those requiring support for chronic illnesses within a hospice framework. You will engage with a diverse range of clients, including older adults and their families, providing empathy and therapeutic interventions that respect their unique cultural and personal backgrounds.

Qualifications
  • Possession of a Master’s degree in Social Work (MSW) or a closely related field.
  • A valid and active License as a Clinical Social Worker (LCSW) in the state of California.
  • At least two years of post-graduate clinical experience, preferably in a healthcare or hospice environment.
  • Proficiency in electronic health record (EHR) systems and relevant software for documenting client interactions and treatment plans.
  • Strong organizational skills and excellent verbal and written communication abilities, with an emphasis on active listening and rapport-building.
