Our client is a New York City-based home care agency providing in-home support services for seniors through a dedicated network of caregivers and home health aides. Role Overview

We are hiring a Remote Registered Nurse based in the Philippines to support the Patient Care department, including NYC-based nursing staff, care coordinators, and a large caregiver network.

This role is heavily focused on caregiver support, care coordination, patient follow-up, and communication across active care cases. The RN will assist with high-acuity client situations, reinforce care compliance, support documentation efforts, and help ensure caregivers remain aligned with care plans and operational expectations.

Key Responsibilities Caregiver & Patient Support
  • Support caregivers with care-related follow-up and reminders
  • Assist with high-acuity client cases under the supervision of NYC-based RNs
  • Help reinforce care plan compliance and patient support protocols
  • Provide responsive communication and support to caregivers during active shifts
Clinical Coordination & Follow-Up
  • Assist the Patient Care Coordinator with documentation and care tracking
  • Support ongoing patient care communication and coordination efforts
  • Escalate urgent or clinically relevant concerns appropriately
  • Maintain accurate and organized records related to patient and caregiver interactions
Communication & Operations
  • Communicate professionally and clearly with caregivers and internal staff
