Mandatory Requirements to Apply

  • Minimum 2 years of experience in health promotion, health education, wellness coaching, or counseling (nutrition, exercise, stress management, chronic-disease prevention).
  • Strong knowledge of diabetes self-management, nutrition, and lifestyle-behavior change.
  • Experience conducting health assessments, goal-setting, and providing telephonic coaching.
  • Skilled in motivational interviewing, patient education, and personalized care planning.
  • Ability to document interactions accurately and comply with healthcare quality standards.


Preferred

  • 2+ years coaching individuals with diabetes or chronic conditions.
  • Bachelor’s degree in Health Education, Health Promotion, Nutrition, Nursing, Social Work, or related field.
  • CDE or CDCES certification required.
