As a Licensed Clinical Social Worker, your responsibilities will encompass a variety of tasks crucial for the holistic care of hospice patients, including:

• Conducting comprehensive psychosocial assessments to identify the emotional, social, and environmental factors affecting patients and their families.
• Developing personalized treatment plans that integrate evidence-based practices with family needs and preferences, prioritizing comfort and dignity.
• Providing therapeutic interventions, including cognitive-behavioral therapy (CBT) and mindfulness practices, tailored to the unique circumstances of patients facing terminal illness.
• Maintaining detailed clinical documentation and case notes in accordance with HIPAA standards to ensure confidentiality and compliance.
• Collaborating actively with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to deliver integrated care that addresses the complex needs of patients.
• Participating in case consultations and peer supervision to continually enhance the quality of care and address team dynamics effectively.
• Educating patients and families on mental health issues, coping strategies, and community resources to empower them during challenging times.

• A Master's degree in Social Work from an accredited program.
• A current and valid Licensed Clinical Social Worker (LCSW) license in the state.
• A minimum of two years of clinical practice with experience in providing psychotherapy services.

• Annual salary ranging from USD 80,000 to 104,000, commensurate with experience and qualifications.
