This role is a remote position, allowing for flexibility and work-life balance while ensuring you can provide quality care from the comfort of your home. You will be an integral part of a remote multidisciplinary team, collaborating closely with healthcare professionals to develop comprehensive care plans for patients in our hospice program.

• Provide individual and family psychotherapy for patients dealing with mental health challenges such as anxiety, depression, and complex grief.
• Conduct comprehensive mental health assessments that evaluate emotional, behavioral, and social functioning, leading to informed treatment decisions.
• Develop and implement individualized treatment plans based on evidence-based practices, ensuring cultural relevance and sensitivity.
• Regularly monitor and evaluate patient progress, making necessary adjustments to treatment plans based on clinical assessments and feedback.
• Collaborate with a team of healthcare professionals, including physicians and psychiatrists, to ensure seamless, integrated care for our hospice patients.
• Provide education and resources to patients and their families regarding mental health issues, available treatment options, and community resources.
• Maintain clear and accurate documentation in electronic health records (EHRs) and uphold ethical standards of practice, including patient confidentiality and informed consent.

This position focuses on patients in hospice care, many of whom are navigating complex emotional landscapes associated with terminal illness and loss. The LCSW will work closely with patients, their families, and caregivers, providing emotional support and interventions designed to enhance their quality of life during an incredibly sensitive time.

• Master's degree in Social Work from an accredited institution, demonstrating a solid foundation in clinical practice.
• Valid and current state licensure as a Licensed Clinical Social Worker (LCSW).
• At least 2 years of experience in providing psychotherapy and mental health services, preferably in hospice or palliative care settings.
• Strong communication and interpersonal skills, enabling the establishment of trusting relationships with patients and families.
• Demonstrated clinical skills, including adeptness in assessments and treatment plan development.
• Proficiency in utilizing electronic health records (EHRs) for effective patient management and communication.
• Deep understanding and awareness of mental health conditions, their treatment options, and how they impact patients' emotional well-being.
• A dedication to maintaining confidentiality and adhering to established ethical guidelines in social work.

• Experience in hospice care or working with terminally ill patients and their families is highly desirable.
• Familiarity with trauma-informed care principles and techniques can enhance therapeutic relationships.
