CRC is widely considered as a visionary in the post-acute space, designing new care models to improve efficiency, decrease hospitalizations, and improve clinical outcomes
Job Responsibilities
You will be responsible for managing the full-cycle recruitment process for Care Coordinator openings across the country to support the organization's workforce needs and growth targets.
You will be required to:
  • Work closely with Care Transitions leadership to understand current and future hiring needs, timelines, and staffing priorities
  • Build and maintain candidate pipelines for Care Coordinator positions across multiple states and markets
  • Source qualified candidates through job boards, resume databases, referrals, social media, professional networks, and direct outreach
  • Develop a strong understanding of the Care Coordinator role, patient population, performance expectations, and ideal candidate profile
  • Review applications and resumes to identify candidates who meet the position's qualifications
  • Conduct initial screening interviews to evaluate candidate experience, communication skills, availability, motivation, and overall fit
  • Assess candidates for their ability to communicate compassionately with patients, manage multiple priorities, and work effectively in a remote healthcare environment
  • Make appropriate candidate recommendations to Care Transitions hiring managers
  • Schedule interviews and maintain timely communication with candidates throughout the hiring process
  • Proactively follow up with candidates and hiring managers to ensure candidates move efficiently through the recruitment funnel
  • Clearly communicate the responsibilities, expectations, compensation structure, benefits, and growth opportunities associated with the role
  • Partner with leadership to coordinate offers, resolve candidate questions, and support offer acceptance
  • Maintain accurate and timely candidate records within the Applicant Tracking System
  • Track recruiting activity, candidate pipelines, interview progress, offers, and hiring outcomes
  • Identify recruiting challenges and recommend strategies to improve candidate quality, hiring speed, and acceptance rates
  • Build relationships with colleges, healthcare programs, professional organizations, and community partners to support future hiring needs
  • Seek referrals from current Care Coordinators and other CRC teammates
  • Support onboarding handoffs by ensuring new hires receive accurate information and complete required pre-employment steps
  • Promote a positive candidate experience that reflects CRC's culture and values

Requirements
  • Bachelor's Degree (preferred)
  • 3+ years of recruitment experience, preferably of full-cycle healthcare recruitment, in a high-growth environment; experience hiring in healthcare strongly preferred
  • 2+ years of experience working successfully in a fully remote environment
  • Experience recruiting Care Coordinators, Patient Care Coordinators, Case Managers, Licensed Vocational Nurses (LVNs), Licensed Practical Nurses (LPNs), Medical Assistants, Patient Service Representatives, or similar healthcare support roles preferred.
  • Strong sourcing skills, including experience using job boards, resume databases, referrals, social media, professional networks, and direct outreach
  • Ability to manage multiple open positions and large candidate pipelines while maintaining timely follow-up
  • Comfortable working in a fast-paced, rapidly changing, and metrics-driven environment
  • Proficient with Microsoft Suite, including Excel, and Google Workspace
  • Excellent (written and verbal) communication and interpersonal skills
  • Ability to collaborate effectively with a distributed team while independently managing responsibilities in a remote environment
  • Past experience of working with databases and Applicant Tracking Systems (preferred)
