You will primarily work from your home office, but will also engage regularly in face-to-face interactions through scheduled home visits, virtual consultations, and community outreach. Part of your responsibilities may require you to adjust your schedule to include occasional evening or weekend hours to meet client needs.

As a Licensed Master Social Worker at Inner Harmony Healing Center, your daily responsibilities will include:

• Conducting comprehensive psychosocial assessments of clients to identify their emotional and social needs.
• Collaborating with clients and their families to create tailored care plans that reflect their individual goals, needs, and preferences.
• Providing counseling to individuals and families to navigate complex life transitions, such as health changes, loss of independence, or bereavement.
• Acting as an advocate for client rights, ensuring they have access to necessary community resources, healthcare services, and support groups.
• Working closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and home health aides, to coordinate and optimize care for clients.
• Conducting regular home visits to monitor the health, safety, and overall well-being of clients, providing intervention as necessary.
• Facilitating communication and decision-making among family members involved in the care process, supporting conflict resolution where needed.
• Educating clients and families about available community services to empower their social engagement and enhance their quality of life.
• Maintaining meticulous documentation of all client interactions, assessments, and care plans in compliance with organizational policies and ethical standards.

Your primary focus will be seniors who may face a range of emotional and sociological challenges, including chronic illness, mental health issues, and changes in family dynamics. Understanding the experiences of this demographic is crucial, as you will address their unique needs through empathetic support and proactive care coordination.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid licensing as a Master Social Worker (LMSW) in the state of Florida.
• A minimum of two years of direct experience in a social work role, with a preference for experience in geriatric care or community settings.
• Robust knowledge of social work principles, aging-related issues, and available community resources.
• Exceptional skills in assessment, counseling, communication, and problem-solving, with a strong emphasis on empathy and rapport-building.
• Proficiency in electronic health record (EHR) systems and a strong familiarity with computer applications relevant to social work practice.

Ideal candidates will have prior experience working in a hospice or comprehensive care environment, with demonstrated abilities to handle sensitive situations with professionalism and compassion. Familiarity with community health systems, aging networks, and a variety of communication strategies will enhance your effectiveness in this role.
