As part of a community health clinic model, you will be engaging with clients through telehealth services, ensuring that mental health care is accessible to all. You will be part of an interdisciplinary team that collaborates to provide comprehensive care, participating in virtual meetings and case consultations to ensure continued support for each patient.

• Conduct ongoing assessments to accurately identify the needs of clients while tailoring interventions to improve their mental health outcomes.
• Facilitate communication among family members, guardians, and other stakeholders to update them on client progress and treatment plans.
• Develop and manage a tentative schedule for follow-up appointments, in-home visits, and progress evaluations to ensure a comprehensive approach to care.
• Coordinate in-home visits in compliance with program requirements, facilitating chart reviews and staff interviews to enhance our counseling approach.
• Monitor the implementation of individualized care plans, ensuring effective execution through regular follow-up and adjustments as needed.
• Participate actively in interdisciplinary team meetings to collaborate with other healthcare professionals, sharing insights and strategies to enhance client support.
• Document monthly and quarterly progress notes reflecting client status and advancements in their mental health journey.
• Lead training sessions to equip staff with the skills necessary for implementing effective program strategies and documentation practices.
• Provide ongoing support through follow-up interviews and home visits to ensure that clients have continuous access to resources and assistance.
• Respond to inquiries from the community and mental health stakeholders, effectively acting as a liaison to foster trust and support.
• Assist in the development of training materials that elevate the quality of care delivered by the team.

In this role, you will work with a diverse clientele, including youth and adults facing a variety of mental health challenges. Our focus is on delivering culturally competent care that is respectful and responsive to the individual needs of each client. You will play a critical role in promoting mental health awareness and providing support for both acute and chronic mental health issues, including but not limited to anxiety, depression, trauma, and substance misuse.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• An active and unrestricted licensed as a Professional Counselor (LPC) in the state of Georgia is mandatory.
• A minimum of two years of counseling experience, preferably in a clinical context, is required.

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse interventions is highly preferred.
• Strong knowledge of legal and ethical standards pertaining to mental health practice.
• Demonstrated abilities in interpersonal communication and establishing rapport with diverse client populations.
• Experience working collaboratively within a healthcare team to enhance holistic client care.
• Familiarity with Electronic Medical Record (EMR) software and technologies relevant to mental health documentation.
