Role Description

Nearly half of Medicare patients require post-acute care after leaving the hospital — and a significant portion of total Medicare spending and avoidable readmissions is driven by what happens in the post-discharge period

As an Arrive Home Nurse Practitioner, you serve as the clinical anchor for homebound patients during the most vulnerable days after discharge.


Through the Arrive Home Program, our Nurse Practitioners provide structured, physician-level oversight to stabilize patients and prevent avoidable readmissions. They do this by:

This is a full-time remote/mobile position for a Nurse Practitioner. As a Nurse Practitioner you will provide high-quality care to patients, including conducting virtual consultations, developing individualized recovery plans, and collaborating with other healthcare professionals and partners. This role also includes monitoring patient progress, adjusting care plans as necessary, and delivering compassionate support to patients and their families.


Qualifications
