• Conduct comprehensive assessments to evaluate seniors' psychosocial needs, including emotional, mental, and social factors.
• Collaborate with clients and their families to develop tailored care plans that align with their personal goals and preferences.
• Provide counseling and emotional support to help seniors navigate significant life transitions and challenges; for example, offer strategies to cope with loss or health-related issues.
• Advocate for clients’ rights and their access to necessary community resources, including support groups and healthcare services.
• Work alongside an interdisciplinary team to ensure coordinated care for clients, participating in team meetings and case discussions.
• Regularly monitor client well-being through home visits, ensuring safe living conditions and identifying any concerns or urgent issues.
• Facilitate family meetings to enhance communication, resolve conflicts, and promote collaborative care strategies.
• Educate clients and families about available community resources, programs, and events that enhance social engagement and improve quality of life.
• Maintain accurate and timely documentation of client interactions, assessments, care plans, and progress notes in compliance with ethical standards.
• Continuously stay informed about current social work practices and regulations, proactively ensuring ethical compliance in service delivery.

This role focuses primarily on providing mental health support to seniors, a demographic that often deals with multiple hurdles, including health declines, loss of independence, and social isolation. As an LCSW, you will be integral in improving their overall well-being and empowering them to lead fulfilling lives. • Master's degree in Social Work from an accredited institution required.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of three years of professional experience in a social work capacity, ideally within a community health setting.
• Comprehensive knowledge of social work principles, aging issues, and community resources pertinent to the senior population.
• Demonstrated proficiency in assessment, counseling, and problem-solving skills that will help adapt to broad client needs.
• Exceptional communication and interpersonal skills, enabling effective teamwork and client engagement.
• Familiarity with electronic health record (EHR) systems and related computer applications for maintaining documentation.

• Experience working with geriatric clients and a thorough understanding of their unique challenges and needs.
• Previous involvement in community outreach or education programs focused on seniors.
• Strong advocacy experience, specifically securing resources for clients in need.

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience and qualifications.
