As a Licensed Clinical Social Worker at Jimenez and Associates, you will:

• Evaluate and assess biopsychosocial needs of patients, addressing a spectrum of psychosocial challenges related to mental and physical health.
• Collaborate with a multidisciplinary team, providing input into comprehensive care planning aimed at enhancing treatment adherence and ensuring a positive patient experience.
• Apply individual and group therapy techniques and evidence-based treatment modalities to develop and implement tailored treatment plans.
• Document all social work-related activities in patient medical records, adhering to departmental standards and the NASW Code of Ethics to maintain integrity in practice.
• Educate patients and their families on treatment processes and available resources, thereby facilitating understanding and compliance with therapeutic recommendations.
• Complete case-related statistics and reports in accordance with organizational guidelines, ensuring data-driven decision-making and quality improvement.
• Stay informed about community resources relevant to patient care, and coordinate advocacy efforts to enhance social support systems in the community.
• Participate in ongoing training and keep your clinical skills sharp by engaging in professional development opportunities.

You will work with a diverse patient population, primarily focusing on individuals facing complex psychosocial issues connected to end-of-life care and chronic illness. The ideal candidate will possess the following qualifications:

• A Master’s Degree in Social Work from an accredited institution, with a current license from the State Board of Social Work Licensure at the LCSW level.

• Exceptional analytical skills to effectively evaluate and strategize treatment interventions.
