Key Responsibilities

• Conduct thorough assessments to understand the social, emotional, and environmental challenges faced by clients, ensuring a comprehensive overview of their unique situations.
• Develop personalized service plans that delineate clear goals, interventions, and strategies for progress monitoring, tailoring your approach to meet individual client needs.
• Provide individual and family counseling, utilizing evidence-based practices to empower clients in overcoming barriers and achieving their goals.
• Offer critical crisis intervention, perform risk assessments, and implement effective safety planning to support clients in urgent situations.
• Actively engage with a network of community agencies to coordinate resources and advocate for clients’ needs, helping them access necessary services such as housing, employment, and healthcare.
• Participate in outreach activities, aimed at raising awareness of social issues and agency services, thereby promoting greater community involvement.
• Contribute to case conferences, collaborate in team meetings, and partake in staff training to enhance knowledge and team synergy.

As a Licensed Independent Clinical Social Worker at Palm Health Medical Center, you will work primarily with individuals experiencing substance use disorders and related mental health issues. Qualifications

• Master’s degree in Social Work from an accredited institution is required for this position.

Compensation and Benefits
