Puzzle Healthcare is seeking a board‑certified Physiatrist (PM&R) to serve as Medical Director, providing clinical oversight and leadership across our post‑acute and long‑term care network. This role blends remote medical direction with periodic on‑site support for complex rehabilitation procedures, making it ideal for an experienced physiatrist who values both flexibility and hands‑on clinical engagement.

As Medical Director, you will guide physiatric care standards, ensure documentation quality and compliance, and collaborate closely with facility teams to elevate rehabilitation outcomes. You will serve as the clinical authority for PM&R across assigned regions, shaping care delivery at scale.

What You’ll Do
  • Provide clinical leadership as the physician-of-record for PM&R services across assigned facilities.

  • Conduct structured remote chart reviews, documentation oversight, and compliance audits.

  • Review and co‑sign clinical encounters per state and payer requirements.
