• Assure patient rights are upheld by advocating for individual needs and ethical decision-making, establishing an environment of trust and respect.
• Implement procedural protocols for reporting vulnerabilities or suspected abuse, adhering to legal mandates to protect patients.
• Participate proactively in the care system’s efforts to reduce readmissions by utilizing technology for risk assessment and intervention.
• Conduct transition conferences that involve patients and families, ensuring smooth shifts in care plans and clear communication of necessary information.
• Employ electronic medical records diligently to track patient progress, enabling informed discussions about treatment efficacy and social work plans.
• Collaborate seamlessly with interdisciplinary teams, anticipating client needs and rallying resources to effectively move care plans forward.
• Provide comprehensive support and counseling for patients and their families, focusing on mental health, patient education, and resource navigation for financial or social support needs.
• Ensure timely referrals and clinical handoffs for at-risk patients, reinforcing their transition plans with collaboration from community and healthcare resources.
• Remain engaged in additional duties as assigned, contributing to the overall functionality and responsiveness of the care team.

In this role, you will work with a diverse client population, including individuals suffering from mental health issues related to acute, chronic, or terminal illnesses. Your focus will extend to family caregivers who often navigate the stresses related to these circumstances, demonstrating a holistic approach to intervention.

• A Master’s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work from an accredited program (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).
• An independent license in the state of residence, such as LCSW, LMFT, or LPC, enabling you to practice without supervision.
• Proficient communication skills, particularly in electronic mail, facilitating clear and timely correspondence with clients, colleagues, and external resources.
• A demonstrated ability to manage a large caseload effectively while maintaining responsive engagement with patients and interdisciplinary teams.

• Previous experience in a similar mental health counseling role is preferred, specifically within outpatient settings.
