Key Responsibilities

  • Maintain an organized schedule for appointments and home visits, ensuring clear strategies for ongoing support and monitoring.
  • Implement and evaluate treatment strategies with fidelity to the program plan, monitoring progress through regular reviews and documentation.
  • Participate actively in Interdisciplinary Team Meetings to ensure cohesive care delivery and address client needs collaboratively.
  • Complete and organize monthly and quarterly progress notes, ensuring documentation is thorough, timely, and compliant with regulatory standards.
  • Provide ongoing support through home visits and follow-up interviews, reinforcing therapeutic goals and monitoring client progress in real-world settings.
  • Act as a community liaison and advocate within the mental health space, collaborating on service development and outreach initiatives.
  • Design and conduct training sessions for staff on effective implementation of treatment protocols and documentation practices.
Patient Population / Client Focus

As an LCPC at Primary Counseling Center, you will work with a diverse patient population, which includes both adults and youth. You will support clients facing a range of challenges, including anxiety, depression, trauma, and relationship issues. Qualifications
