• Provide psychosocial assessments and develop individualized treatment plans that reflect the specific needs of patients and their families.
• Utilize electronic medical records effectively to monitor patient progress, document interactions, and communicate updates with the interdisciplinary team.
• Work collaboratively with healthcare providers to anticipate patient needs and adjust care plans accordingly, ensuring a holistic approach to patient care.
• Offer support and resources to patients and families regarding transition plans during their journeys through care.
• Stay informed about healthcare networks and payer services to facilitate smooth transitions within the patient care continuum.
• Advocate for patients by ensuring their rights are upheld and connecting them to necessary protective services as mandated.
• Participate in care coordination and community resource collaborations to enhance patient support systems.
• Engage with patients who exhibit a risk for readmission, utilizing technology and assessment tools to prevent potential crises.
• Ensure the ethical handling of sensitive information and reporting of vulnerabilities, including suspected abuse, as required by law.
• Conduct transition conferences with patients and families to facilitate effective handoffs and continuity of care.
• Fulfill additional duties as assigned, contributing to the overall effectiveness of the mental health team.

This role primarily serves a diverse patient population, including individuals dealing with significant mental health challenges stemming from acute, chronic, or terminal conditions. You will work closely with patients and their families, empowering them with knowledge, support, and coping strategies as they navigate complex emotional landscapes.

• A Master’s or Doctorate degree in clinical psychology, counseling, or social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• A valid independent license in the state of Oregon (e.g., LCSW, LMFT, LPC) is required.
• Strong organizational skills and the ability to manage a large caseload effectively.
• Proficient in the use of electronic medical records and email communication.

• Demonstrated passion for impacting lives within the behavioral health field, with experience providing comprehensive counseling services.
• Familiarity with the healthcare landscape, including government and private payers, significantly enhances candidate suitability.
• Previous experience working in community health settings is an advantage, particularly in remote care practices.
