Position Overview

Key Responsibilities

• Assist primary care providers in recognizing and addressing mental health and psychosocial issues, serving as a valuable member of the primary care team.
• Conduct patient intakes with a focus on diagnostic evaluations, providing recommendations for treatment plans to primary care providers.
• Deliver brief, targeted interventions for patients requiring mental health services, following evidence-based practices for therapy.
• Monitor and evaluate patients’ progress, initiating follow-up sessions to adjust treatment approaches as necessary.
• Develop and implement relapse prevention plans, supporting patients in maintaining stability and functional capacity.
• Guide the care team in identifying at-risk patients, creating plans to prevent deterioration of mental health conditions.
• Document all patient interactions, treatment plans, and progress notes thoroughly and accurately in electronic health records.
• Collaborate closely with primary care providers to formulate and adjust treatment strategies based on patient needs.
• Empower patients through education on self-management skills and coping strategies, fostering their active participation in recovery.
• Provide consultation to clinic management on behavioral health services, contributing to program evaluation and improvement efforts.

Qualifications

• A Master’s Degree in Clinical Counseling or a related field is required.

• Ability to work effectively within a multidisciplinary team, fostering collaborative relationships with colleagues and patients.

Preferred Experience

• Experience working in outpatient settings, especially within Federally Qualified Health Centers (FQHC).
• Knowledge of bilingual communication, specifically Spanish, is preferred but not mandatory.
• Familiarity with community resources and the ability to connect patients with appropriate services.
• Expertise in various therapeutic modalities, including family therapy and child psychotherapy.

Compensation and Benefits

Yakima Family Health Services offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
