Key Responsibilities
  • Conduct comprehensive assessments and ongoing evaluations to tailor mental health interventions to meet the unique needs of each client.
  • Maintain open communication channels with supervisors, healthcare team members, and clients' families to track progress and treatment modifications.
  • Organize and present a tentative schedule of appointments and home visits to maximize client engagement and therapeutic progress.
  • Coordinate necessary home visits for in-home observations, ensuring compliance with program requirements while providing support and guidance to clients in their familiar environments.
  • Monitor the implementation of individualized treatment plans to ensure efficacy and adherence to therapeutic goals.

  • Document monthly and quarterly progress notes, maintaining clear and accurate records that are accessible to relevant parties.
  • Provide additional follow-up support through interviews and home visits, reinforcing treatment goals and client success.

  • Establish yourself as a liaison within the mental health community to promote development goals and foster positive relationships.
Patient Population / Client Focus
