Work Environment

This role operates in a remote setting, providing you with the flexibility to manage your schedule while still delivering high-quality care. The position is primarily home-based, allowing you to conduct client visits and assessments in a variety of residential settings. While you will collaborate closely with a team of healthcare professionals, much of your communication and coordination will be done virtually, utilizing secure telehealth platforms.

Key Responsibilities

• Conduct detailed assessments of seniors' psychosocial needs, including evaluating emotional health, social support systems, and environmental factors influencing their well-being.
• Collaborate with clients and families to design individualized care plans that reflect their unique goals, values, and preferences, adapting as their needs evolve.
• Offer compassionate counseling and emotional support, helping clients navigate challenges such as health transitions or adjustment to aging.
• Act as a strong advocate for clients, ensuring they have access to community resources, educational programs, and services that align with their needs.
• Work alongside interdisciplinary teams, including nurses and home health aides, to coordinate comprehensive care for clients, enhancing their overall health outcomes.
• Regularly visit clients in their homes to provide ongoing support and reassess their needs, while identifying any new challenges or safety concerns.
• Educate clients and their families on available community services to promote engagement and facilitate connections within their communities.

Qualifications

• Bachelor's degree in Social Work from an accredited institution is required, with a focus on geriatric social work preferred.
• Current licensure as a Social Worker in Arizona (LCSW, LMSW, or equivalent).
• Strong foundation in social work methodologies, particularly in home care or geriatric settings.
• Exceptional communication and interpersonal skills, with the ability to connect deeply with clients and families.

Compensation and Benefits
