As a Licensed Mental Health Counselor, your day-to-day responsibilities will include:

• Conducting thorough assessments of new patients to gather historical health data and understand individual mental health needs.
• Designing, implementing, and periodically reevaluating individualized care plans based on established clinical standards.
• Engaging patients and their families through effective interviews to identify educational needs and collaboratively set achievable treatment goals.
• Monitoring patient progress and adjusting care plans as necessary to ensure optimal outcomes.
• Coordinating with other healthcare professionals, including physicians and social workers, to facilitate comprehensive care and effective communication regarding patient status.
• Ensuring meticulous documentation of patient interactions and progress in accordance with HIPAA regulations and organizational policies.
• Conducting engaging and informative workshops for groups, aimed at improving mental health awareness and strategies for emotional well-being.
• Providing referrals to other specialists or maintaining contact with external care providers when needed to enhance patient support.

Your expertise will primarily support cardiac patients enrolled in our targeted mental health program. You’ll utilize your knowledge of mental health issues to address the unique psychological challenges faced by patients with cardiac conditions, helping to improve their overall quality of life. By managing mental health alongside physical health, you play a crucial role in their recovery journey.

The ideal candidate will possess the following qualifications:

• A Master's degree in Counseling, Psychology, Social Work, or a related field.
• A valid and unrestricted Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Psychologist license.
• An understanding of HIPAA regulations and clinical documentation standards.
• Strong communication skills to effectively facilitate individual and group therapy sessions.
