• Engage in regular communication with physicians and other healthcare professionals to report on patient progress, address needs and concerns, and troubleshoot potential issues affecting client care.

• Consult with families and support networks regarding treatment plans and assist in effective discharge planning to ensure continuity of care for clients.

• Maintain precise and up-to-date patient records, including clinical notes and progress updates, adhering to all departmental policies and legal requirements.

• Collaborate with multidisciplinary healthcare teams to develop tailored treatment plans utilizing evidence-based social work interventions based on thorough psychosocial assessments.

• Conduct discharge planning and care coordination for clients, working closely with various community and healthcare providers to facilitate access to necessary services and resources.

The clients served by Regenerate Wellness Clinic encompass a broad spectrum of individuals facing mental health challenges, including anxiety, depression, trauma, and substance-related disorders. As a Mental Health Counselor, your expertise will be essential in assessing their needs and providing personalized therapeutic interventions aimed at helping them achieve their recovery goals.

• Master's degree in Social Work, Counseling, Marriage & Family Therapy, Psychology, or a closely related field.

• Active state licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or equivalent.

• A minimum of three years of experience working in an outpatient mental health setting, demonstrating a strong clinical foundation.

• Proficiency with computer programs, including word processing software, email systems, spreadsheets, and electronic medical records to effectively document and manage patient information.

• Ability to work independently, demonstrating self-motivation and exceptional organizational skills to manage a diverse caseload effectively.

While not mandatory, candidates with experience in teletherapy, group therapy facilitation, or working with specific populations (such as adolescents or substance abuse clients) will be viewed favorably. Familiarity with culturally competent practices and community resources in Chester, Pennsylvania, is also beneficial.
