As a Remote LCSW, you will be part of a collaborative and supportive virtual team committed to enhancing quality of life for patients in hospice care. Your work will primarily take place through telehealth sessions, allowing for significant flexibility in your schedule. The role includes interactions with a diverse patient population, as well as ongoing communication with other members of the healthcare team, including physicians, nurses, and volunteers.

• Conduct thorough psychosocial assessments to evaluate clients' mental health needs and challenges.
• Provide tailored therapeutic interventions, including individual, family, and group therapy, utilizing evidence-based practices such as cognitive-behavioral therapy and solution-focused therapy.
• Develop and implement personalized treatment plans that address the unique needs of each patient and their families, facilitating goals and therapeutic outcomes.
• Engage in regular collaboration with the medical team to ensure comprehensive care coordination and effective treatment strategies.
• Offer expertise in crisis intervention, including risk assessment, safety planning, and emotional support during critical moments.
• Maintain meticulous documentation in compliance with all relevant ethical and legal standards, ensuring real-time updates to care plans.
• Facilitate group therapy sessions and support groups, encouraging community building among patients and families.

Your focus will be on patients facing complex and often terminal diagnoses, requiring a sensitive approach that considers the emotional weight of their circumstances. Families navigating the challenges of hospice care will also be a central part of your clientele, as you provide education and support to help them manage their loved one's journey.

• A Master’s degree in Social Work from an accredited program.
• Current, active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a mental health or outpatient setting, particularly with a focus on hospice or palliative care is preferred.

• Demonstrated knowledge of evidence-based therapeutic modalities and approaches tailored to hospice care.
• Strong interpersonal, assessment, and communication skills, particularly in navigating delicate conversations.
• Experience working collaboratively within a multidisciplinary healthcare team.

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience.
