As a Remote LCSW, your day-to-day responsibilities will include:

• Conducting thorough psychosocial assessments to evaluate clients’ mental health concerns and strengths in the context of hospice care.
• Developing customized, evidence-based treatment plans tailored to each client’s unique needs, addressing symptoms of anxiety, depression, grief, and complex trauma.
• Providing a range of therapeutic interventions, such as individual and family therapy, to promote mental well-being during challenging times.
• Facilitating group therapy sessions and psychoeducational classes aimed at helping families navigate grief and loss.
• Collaborating closely with a team of healthcare professionals, including physicians and psychiatrists, to ensure comprehensive, coordinated care.
• Maintaining meticulous and timely documentation of client interactions, including treatment plans and progress notes, in compliance with ethical and legal standards.
• Engaging in crisis intervention and risk assessment as necessary, ensuring that clients are supported effectively during critical moments.

Our clients include individuals within a hospice setting who are navigating terminal illness or caregiving responsibilities. You will work with diverse patients from various backgrounds, each facing unique emotional and psychological challenges due to their circumstances. Your empathetic and holistic approach will be crucial in providing them with the mental health support they need.

We require candidates to possess the following:

• A Master’s degree in Social Work from an accredited program.
• An active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in mental health treatment within outpatient or hospice settings, demonstrating a solid grounding in psychosocial support.
• Proficient knowledge of evidence-based therapeutic modalities, with a strong emphasis on cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care approaches.
• Excellent interpersonal, communication, and conflict-resolution skills, with a strong ability to connect with clients compassionately.

While not mandatory, the following experience will be advantageous:

• Experience working within a hospice or end-of-life care setting, understanding the unique challenges involved.
• Familiarity with telehealth platforms and virtual therapeutic practices that enhance client engagement.
• Experience in multidisciplinary case collaboration, contributing effectively to interdisciplinary teams.
