About the Job

The Care Coordinator, in collaboration with the multidisciplinary provider team, is responsible for the timely coordination of health care services to meet every patient\'s individual needs and promote high quality visit experiences and cost effective clinical outcomes.

Medical Assistant Certification is required.

This is a full-time benefited position with a 90 day probation period.



Key Functions & Responsibilities (included But Not Limited To):

Patient Support

  • Initiate proactive patient outreach to support lab/imaging/referral tracking, gaps in care and other quality measure improvement efforts.
  • Perform multiple administrative tasks such as; Scheduling appointments, sending out lab/imaging orders, assisting with pharmacy calls and prescriptions, completing referrals, updating electronic health records, adding/updating insurance packages, collecting payments.

Communication

  • Demonstrate empathy and professionalism via all communication methods, including: phone, video, email, slack, text, and patient portal messages
  • Conduct effective and compassionate communication with patients to plan, implement and follow up on patient needs.

Problem Solving

  • Real time clinical support to solve/ trouble shoot challenges that come with providing remote virtual services.
  • Assisting patients with lab, imaging or pharmacy issues in real time.
  • Identifies and appropriately manages patient related issues to the Provider when required.
  • Escalates to Practice Leadership in a timely manner when patient cases require special or immediate attention. (E.g. adverse events or patient complaints) 

Following Regulations 

  • Provides quality customer service- follows HIPAA rules/regulations.
  • adhere to all relevant regulations, policies, and procedures to ensure patient safety and privacy
  • Adhere to internal compliance program/protocols 



Requirements

Qualifications

  • 2-3 years of healthcare customer service experience with hybrid practice models
  • Medical Assistant certification required
  • Prior experience utilizing the CoverMyMeds system required
  • Demonstrated excellence navigating and using an EMR 
  • Must be willing to work nights or weekends including east/west coast time zones
  • Highly organized, detail-oriented, self-starter
  • Ability to work Tuesday through Saturday 7:00am - 4:00 pm Pacific Standard Time or 10:00am - 7:00pm Eastern Standard Time
