Key Responsibilities
  • Conduct comprehensive mental health assessments through detailed psychosocial evaluations and diagnostic interviews, diligently identifying patient needs and potential risks for intervention.
  • Partner with patients to create tailored treatment plans that reflect their individual goals, life experiences, and cultural backgrounds, ensuring that all care delivered is respectful and appropriate.
  • Utilize a range of evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), using these methods in individual and group therapy sessions.
  • Deliver crisis intervention services as needed, working closely with other healthcare professionals to ensure seamless continuity of care, especially for patients experiencing acute distress.
  • Maintain meticulous clinical documentation in accordance with the clinicโ€™s policies and regulatory standards, including progress notes and treatment modifications.
  • Educate patients and their families on mental health conditions, equipping them with the knowledge and skills necessary to manage their situations effectively.
  • Engage actively with a multidisciplinary team of psychiatrists, psychologists, nurses, and case managers to provide integrated care and optimal outcomes for patients.
  • Regularly conduct outcome assessments to gauge the effectiveness of treatment and make necessary adjustments to improve care plans.
  • Stay updated on the latest research and best practices in mental health to continuously improve therapy techniques and contribute to the teamโ€™s collective knowledge base.

Patient Population / Client Focus

The LCSW will serve a diverse client population within the community health clinic, including individuals experiencing various mental health challenges. Qualifications
  • A Masterโ€™s degree in Social Work (MSW) is required.

Compensation and Benefits
