As a Social Worker, you will have the flexibility to schedule visits and communicate with clients via virtual platforms. While the work is primarily remote, regular meetings with our interdisciplinary teams, including healthcare professionals, will occur to ensure a holistic approach to senior care.

• Conduct comprehensive assessments of seniors' psychosocial needs, factoring in emotional, mental, and social influences to create a detailed understanding of each client's situation.
• Collaborate closely with clients and their families to craft personalized care plans that align with their specific goals, preferences, and available resources.
• Provide compassionate counseling and emotional support to navigate life transitions such as health changes, loss of independence, and relocation.
• Serve as an advocate for clients, helping them access community services, legal rights, and other beneficial resources tailored to their unique circumstances.
• Engage in regular monitoring visits to assess the ongoing well-being of clients, evaluate their living conditions, and address any concerns regarding safety or emotional distress.
• Facilitate effective communication within families and among professional caregivers to ensure comprehensive support for the client, resolving potential conflicts and fostering a collaborative environment.
• Educate clients and families about community resources, support groups, and programs to enhance their social engagement, promoting overall well-being.

• Master's degree in Social Work from an accredited institution.
