Work Environment

This position is remote, allowing you the flexibility to manage your schedule while fulfilling your responsibilities to our clients. While you will primarily interact digitally, occasional on-site visits may be required to conduct assessments or provide counseling directly in the school environment. You will work from the comfort of your own home but should remain accessible for on-call support as needed for urgent client situations. The office hours are maintained from Monday to Friday, 8:00 AM to 4:30 PM, with potential additional hours based on client needs.

Key Responsibilities
  • Conduct in-depth psychosocial assessments to understand the needs of seniors, considering their emotional, mental, and social dimensions.
  • Collaborate closely with clients, their families, and educators to create individualized care plans that respect the seniors' goals and preferences.
  • Provide counseling to seniors and their families, helping them navigate transitions such as health changes, loss of independence, or bereavement.
  • Act as an advocate for clients, ensuring they receive support and access to essential community resources like healthcare services and support groups.
  • Engage with an interdisciplinary team, including nurses and caregivers, to coordinate care effectively and enhance the delivery of services.
  • Monitor client progress, address concerns regarding their living conditions, and ensure their safety during regular visits.
  • Facilitate communication among family members involved in a senior client's care, mediating any conflicts that may arise regarding care decisions.
  • Keep meticulous and timely documentation of all client interactions, assessments, and progress notes in electronic health records systems.
  • Stay abreast of the latest social work practices and regulations, ensuring compliance with ethical standards and best practices in care.
Patient Population / Client Focus

The primary clientele for our services are seniors, often facing unique life challenges associated with aging. These may include health-related issues, social isolation, emotional distress, and transitions associated with aging. Your role as an LCSW will directly contribute to improving their mental health, emotional stability, and engagement with their community.

Qualifications
  • A Master's degree in Social Work from an accredited university is required.
  • Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
  • A minimum of two years of relevant experience in social work, ideally within a community setting or with geriatric populations.
  • Strong understanding of social work principles, aging issues, and available resources in the community.
