Life Stage Counseling is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team as a full-time remote professional. Work Environment

This position is fully remote, offering flexibility to meet the needs of our clients in a compassionate and efficient manner. While the primary work setting is at home, you may occasionally visit clients in their residences or community facilities to provide in-person support and ensure comprehensive care.

Key Responsibilities
  • Conduct in-depth assessments of seniors' psychosocial needs, taking into account their emotional, mental, and social circumstances.
  • Work collaboratively with clients and their families to create tailored care plans that align with their goals and preferences.
  • Provide counseling and emotional support in navigating life changes, ensuring clients feel heard and understood.
  • Advocate for clients' rights, ensuring access to community resources that promote health and well-being.
  • Monitor and assess clients' living conditions through regular virtual check-ins and occasional home visits.
  • Facilitate effective communication among family members involved in the care process, diffusing potential conflicts.
  • Educate clients and their families about available community services and support groups to enhance social engagement.
  • Maintain comprehensive records of client interactions, assessments, and care plans, adhering to ethical and professional standards.
  • Stay current with developments in social work practices, ensuring compliance with relevant policies and regulations.
Patient Population / Client Focus

The primary clientele for this role includes seniors facing a myriad of challenges related to aging, such as health issues, social isolation, family dynamics, and transitions in living situations. You will be instrumental in identifying and addressing their unique psychosocial needs, guiding them towards improvements in their quality of life.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Master Social Worker (LMSW).
  • Minimum of two years of relevant social work experience, preferably in a geriatric or community health setting.
  • Deep understanding of aging-related issues and resources available to the senior population.
  • Outstanding assessment, counseling, and advocacy skills.
  • Ability to demonstrate empathy and compassion in building rapport with clients and families.
  • Exceptional communication skills for effective collaboration with interdisciplinary teams.
  • Familiarity with electronic health record (EHR) systems and other digital tools.
Preferred Experience
  • Experience working in home health care or community health settings.

Compensation and Benefits
