This position is designed for a full-time remote work setting, primarily serving clients engaged in our detox center program located in Northampton, Pennsylvania. This flexibility allows our Licensed Clinical Social Worker to effectively manage their caseload while providing essential support to patients in the comfort of their homes. You will utilize telehealth platforms and electronic health records (EHR) to maintain seamless communication, ensuring that you remain connected with both patients and the care team.

• Provide comprehensive case management services to a select caseload of children and families, ensuring personalized attention and effective support.
• Triage and assess client needs promptly, responding swiftly to emerging issues or required interventions.
• Guide families through the completion of essential forms and applications, empowering them to access necessary services.
• Collaborate with families to identify and address social determinants of health (SDOH) that impact their overall well-being.
• Connect families to relevant community resources, fostering a support network that aids in their journey towards recovery.
• Monitor referrals and follow up diligently to guarantee the delivery of services and resources.
• Assist patients with scheduling and attending appointments with medical professionals, behavioral health specialists, and other relevant parties.
• Provide comprehensive administrative support, including managing records retrieval, eligibility screenings, and goal tracking.
• Coordinate all necessary care for patients, ensuring a collaborative approach to treatment.
• Conduct home and community visits as needed to assess clients' situations and provide direct support.
• Collaborate closely with the interdisciplinary care team, including nursing staff, medical providers, and external partners to ensure cohesive patient care.
• Maintain accurate documentation of all patient interactions and services rendered via the electronic health record system.
• Engage in internal and external meetings, contributing to case conferences and administrative discussions.
• Participate in ongoing training workshops that focus on enhancing health coaching and engagement techniques.
• Provide timely and accurate reporting of data as required for care management programs.
• Offer insights and recommendations for case management services and quality improvement initiatives.

This role primarily focuses on serving clients at our detox center, where you will work with individuals facing substance use disorders and their related mental health issues. The patient population includes children and families, highlighting the need for sensitivity, resilience, and hope throughout the recovery process. Your efforts will directly influence their paths toward stability and improved health outcomes by addressing complex needs holistically.

• Must hold an active LCSW license in Pennsylvania.
• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• A minimum of two years of clinical experience within mental health or social services is essential.
• Proficiency in evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or Eye Movement Desensitization and Reprocessing (EMDR) is preferred.
• Demonstrated skills in case management, assessment, and crisis intervention.
• Exceptional communication, organizational, and teamwork abilities to collaborate effectively with diverse stakeholders.

• Experience working within detox or substance abuse settings will be highly regarded.
• Familiarity with community resources in Northampton, PA, and the surrounding areas.
• Knowledge of trauma-informed care practices and experience with vulnerable populations.
