• Conduct comprehensive psychosocial assessments for patients and their families in a hospice setting, identifying needs, challenges, and resources.
• Develop personalized treatment plans that encompass emotional support, counseling, and advocacy to enhance patient comfort and quality of life.
• Implement solution-focused therapeutic interventions, including grief counseling and crisis intervention, tailored to individual circumstances.
• Document and maintain accurate, timely, and confidential records in accordance with HIPAA regulations and organizational standards.
• Collaborate closely with a multidisciplinary treatment team, which may include physicians, nurses, and chaplains, to coordinate comprehensive care.
• Stay informed on the latest research and best practices in social work and hospice care, integrating evidence-based approaches into your practice.

The primary focus of this role will be on patients in hospice care, including those dealing with terminal illnesses such as cancer, heart disease, and neurodegenerative disorders. You will work with diverse populations, offering culturally sensitive support to patients and their families as they navigate the complex emotional and psychological aspects of end-of-life care.

• A Master’s degree in Social Work from an accredited institution.
• Current LCSW (Licensed Clinical Social Worker) license in Pennsylvania.
• Strong communication and interpersonal skills, with the ability to build trusting relationships with patients, families, and colleagues.
• A proactive problem-solver who can adapt interventions based on individual patient needs.
• Experience in hospice or end-of-life care is highly preferred.

Previous experience in a hospice setting or with palliative care is highly advantageous. Familiarity with trauma, anxiety, and depression within the context of terminal illness will also be beneficial to thrive in this role. Additionally, skills in using electronic health record systems to document and track patient care are preferred.
