• Conduct thorough clinical assessments of patients' mental health needs and create individualized treatment plans that align with their unique circumstances.
• Facilitate evidence-based therapeutic interventions, including individual psychotherapy and family counseling, designed to improve the mental health of patients facing terminal illnesses.
• Maintain accurate and timely clinical documentation in accordance with legal and ethical standards, ensuring confidentiality and effective communication with the interdisciplinary team.
• Engage in continual collaboration with a multidisciplinary team, including physicians, nurses, and other therapists, to provide holistic care to patients.
• Educate patients and their families about mental health issues, treatment options, and coping strategies to foster understanding and resilience during challenging times.
• Stay informed about the latest developments in mental health research and methodologies, applying this knowledge to improve clinical practices.

You will work closely with a diverse patient demographic experiencing end-of-life challenges, mental health conditions, and emotional distress related to terminal illnesses. Prioritizing empathy and understanding, you will address each patient’s needs holistically, ensuring that both patients and their families receive the emotional support necessary during such critical life stages.

• A Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Pennsylvania.
• Strong knowledge of and experience in solution-focused therapies and mental health interventions.
• Exceptional communication, interpersonal, and problem-solving skills.

• Experience in hospice care, palliative care, or working with clients in end-of-life scenarios is highly desirable.
• Familiarity with digital therapy platforms and virtual clinical documentation practices.
• Experience working in a team-oriented environment that promotes collaborative care.
