Trillium Health Center in Frederick, Maryland, is seeking a compassionate and driven Licensed Clinical Social Worker (LCSW) for a full-time remote position within our Hospice Center. This position is fully remote, allowing for flexibility while maintaining a focus on providing exceptional care to our clients. As a part of a closely-knit team, you will be working collaboratively with other healthcare professionals, including doctors, nurses, and administrative staff, facilitating an environment of support and shared goal-setting—all aimed at enhancing patient outcomes.

• Directly engage with clients suffering from major mental illnesses and severe emotional disorders through virtual consultations and community outreach.
• Participate actively in team meetings to discuss case management and treatment strategies, contributing insights to achieve collective health goals.
• Work inclusively with individuals from diverse backgrounds, respecting various cultural traditions and interpersonal styles.
• Familiarize yourself with and navigate the organizational and financial frameworks of Trillium and associated healthcare systems to ensure streamlined service delivery.
• Uphold the utmost standards of professional ethics and the COHC code of conduct in all interactions and decisions.
• Conduct comprehensive assessments and screenings for clients, utilizing evidence-based methods tailored to individual developmental needs.
• Create and implement integrated care plans designed to connect clients with necessary services while facilitating clear communication among clients, families, and providers.
• Provide essential prevention, treatment, and recovery services, including individual, group, and family therapy sessions, tailored to the needs of your caseload.
• Engage in crisis interventions as necessary, ensuring immediate support for clients in acute distress.
• Maintain accurate documentation and ensure the timely submission of reports while monitoring treatment milestones from admission to discharge.
• Participate in on-call coverage as assigned, ensuring that clients receive timely support during and after program hours.
• Collaborate in committees dedicated to enhancing service delivery and overall agency quality improvement initiatives.

This role centers on supporting clients facing complex emotional and psychological challenges, particularly within the realm of hospice care. You will work with clients experiencing terminal illnesses and their families, providing essential psychological support during an incredibly vulnerable time. Your ability to foster trust and create a supportive environment is fundamental to helping clients manage their symptoms effectively and achieve a sense of peace and dignity during challenging transitions.

• A Master’s degree in Social Work (MSW) or a related field is mandatory for this role.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Maryland is required.
• Demonstrated understanding of mental health diagnoses, treatment interventions, and crisis management.
• Excellent written and verbal communication skills are essential for effective client interaction and documentation.
• Strong organizational skills to manage caseloads efficiently and meet reporting deadlines.
• A proven track record of working effectively with diverse populations and adapting techniques to meet their unique needs.
• Competency in developmental and age-related issues pertinent to the client demographics served.

• A minimum of two years’ clinical experience is beneficial and preferred.
• Bilingual proficiency in English and Spanish is highly desirable and will enhance communication with a broader client base.

This position offers a competitive salary range of USD 80,000 – 100,000 per year, along with a sign-on bonus. Our comprehensive benefits include:
