This position offers a flexible remote work environment. Although your primary work will occur virtually, you will also engage with clients and other healthcare professionals in the community and hospice settings, ensuring comprehensive and accessible care. • Provide direct clinical services to assigned clients experiencing major mental illnesses and severe emotional disorders, establishing effective therapeutic relationships.
• Conduct comprehensive assessments, develop, and implement individualized care plans, ensuring that clients access necessary support and resources.
• Facilitate individual, group, and family therapy sessions, adapting approaches to meet the needs of a diverse clientele.
• Engage in crisis intervention efforts as necessary, ensuring the safety and wellbeing of clients in high-stress situations.
• Monitor and document client progress and outcomes diligently, including compliance with treatment plans and actions taken during updates.
• Participate in team meetings and case conferences to discuss client progress, treatment strategies, and collaborative approaches to care.
• Maintain adherence to professional ethical guidelines and the organization’s code of conduct while also identifying and reporting unsafe situations.
• Assist in the development of community partnerships to enhance service delivery and improve client outcomes.
• Provide on-call support as needed, ensuring continuous availability to clients and team members during and after program hours.
• Contribute to agency committees aimed at enhancing service delivery and quality assurance measures.

The clients you will serve are individuals and families navigating complex emotional and mental health issues within a hospice environment. This often includes high-stakes conversations regarding end-of-life care, grief, anxiety, and various mental health crises. The nature of this work requires sensitivity, respect, and a commitment to addressing not only the mental but also the emotional and social needs of clients and their families.

• A valid and current LCSW license in the state of California is required.
• A Bachelor’s degree in Social Work or a related field is mandatory; a Master’s degree is preferred.
• Minimum of two years of relevant clinical experience, particularly within mental health and social work fields.
• Proficiency in electronic health record systems and clinical management software.
• Strong communication skills, allowing effective interaction with clients, families, and multidisciplinary teams.
• Commitment to working within underserved communities and an understanding of the social determinants of health.

• Experience working with hospice patients or within end-of-life care settings.
• Familiarity with evidence-based practices in social work, psychotherapy, and crisis intervention methodologies.
• Understanding of cultural competency in mental health services delivery.

The annual salary for this position ranges from USD 70,000 to 85,000, depending on experience and qualifications.
