Key Responsibilities
  • Conduct thorough psychosocial assessments to evaluate clientsโ€™ mental health concerns, identifying strengths and challenges.
  • Provide tailored individual and family therapy, employing evidence-based practices such as cognitive-behavioral therapy, motivational interviewing, and trauma-informed care.
  • Develop comprehensive treatment plans that encompass client-specific goals, interventions, and ongoing progress monitoring to ensure effective care.
  • Collaborate proactively with a multidisciplinary hospice care team, including medical professionals, to design integrated care strategies that address both physical and emotional health needs.
  • Provide crisis intervention, managing risk assessments and safety planning for clients facing urgent emotional challenges.
  • Advocate for clientsโ€™ rights and connect them with necessary community resources, supporting their overall well-being through case management.
  • Engage in community outreach efforts to promote hospice services, raise mental health awareness, and build strong community networks.
Patient Population / Client Focus

This role centers around a diverse clientele, primarily focusing on individuals and families experiencing end-of-life challenges. You will work with clients from various cultural backgrounds, necessitating an understanding of different cultural perspectives on death, grief, and care. Your role as an LCSW will involve providing emotional support and facilitating communication among family members during this crucial time.

Qualifications
  • Master's degree in Social Work from an accredited institution.
